Providers:
INSTRUCTIONS FOR INJURY CLINIC
Who: Student athletes with acute injuries. How: Parent, coach, or athletic trainer MUST come with an athlete. If a minor, parental attendance is strongly encouraged. Parental consent will be required for all minors accompanied by someone other than a parent/legal guardian (i.e. athletic trainer, coach, sibling, etc.)
Cost: Complimentary for the initial evaluation only (includes X-rays). However, we ask athletes to bring insurance cards and/or school claim forms in case further studies other than X-rays (i.e. MRI, CT scans) or durable medical equipment (i.e. braces, crutches) are needed.
Rehab: Rehabilitation will be provided by the school’s athletic trainer when available and when applicable. When athletic trainers are not available, physical therapy may be completed at local facilities.
